Witness Statement Form

Vantage Point Questions

Question Purpose

Narrative Account

Instructions to witness: Describe what you saw and heard in your own words. Focus on facts—what was said, what actions were taken. Avoid speculation about intent or motivation.

Provide open space for:

  • Timing between events
  • Other witnesses present
